Physicians

A Medicare ACO is a network of doctors, hospitals, and other health care providers who work together with Medicare to give Medicare Fee-For-Service Beneficiaries better service and care. Medicare ACOs focus on quality, cost and the overall care of Medicare Fee-For-Service Beneficiaries. Specifically, they:
  • Ensure coordination of patient care
  • Address chronic diseases
  • Improve the quality of care
 
Medicare Fee-For-Service Beneficiaries are eligible to benefit and use our resources at no cost.  You can continue to bill directly to Medicare for the services and care you provide to Medicare Fee-For-Service Beneficiaries.
 
A Medicare ACO is not the same as a Medicare Advantage plan or a HMO. Your IPA or Medicare Advantage patients are not eligible to participate in our Medicare ACO. However individuals with Fee-for-Service Medicare as supplemental insurance can participate.

Medicare will measure our quality of care in the following areas:
·         Patient/caregiver experience (7 measures)
·         Care coordination/patient safety (6 measures)
·         Preventative health (8 measures)
·         At-risk population
o   Diabetes (6 measures)
o   Hypertension (1 measure)
o   Ischemic Vascular Disease (2 measures)
o   Heart Failure (1 measure)
o   Coronary Artery Disease (2 measures)
